Submitting an online request does not establish a doctor-patient relationship until your request is confirmed by our clinical staff.

By opting in to SMS communications via our candidate quiz or online forms, you consent to receive consultation reminders and updates from Southwest Ocular Care.

You may cancel the SMS service at any time by texting STOP to our number. After you send STOP, we will send a one-time SMS confirming that you have been unsubscribed. Reply HELP for support instructions.

Standard message and data rates may apply for any messages sent to you from us and to us from you. Message frequency varies.

Carriers are not liable for delayed or undelivered messages.

Southwest Ocular Care receives a non-transferable, revocable license to use these proprietary systems solely for the duration of its active service subscription ($997–$4,997 per month). Upon termination or lapse of the subscription, all rights to use, display, or operate these custom systems cease immediately, and the underlying assets may be deactivated or repurposed by the platform provider.

You acknowledge that the proprietary systems described above are provided as part of a commercial software-as-a-service arrangement and are not sold, assigned, or otherwise transferred to you or any end user.
